Will you send materials for inclusion in the attendees' conference bags (approximately 200 pieces)?
Will you need an internet connection at your table? Separate arrangements will need to be made with the hotel unless you are a premium sponsor. Electricity and Internet connectivity will be provided upon request (at no additional cost) for sponsors at the Diamond, Platinum and Gold levels.

Upon receipt of this form, you will be contacted regarding payment. Before you receive any sponsor
benefits, full payment must be received.
